Two Exams per year:
- New patient/comprehensive exam; or
- Periodic exam; or
- Limited oral exam
Cleaning & Preventative Care
- Adult Cleaning (two per year); or
- Perio Maintenance (two per year); or
- Child Cleaning (two per year)
Dental X-Rays:
- Panorex (one every three years of membership); or
- Full Mouth X-Ray Series (one every three years of membership); or
- Bitewings (one set per year); or
- Periapicals (as needed and determined by doctor)
